- Symptoms management for cancer and non-cancer patients including complex symptoms needs
- Provide quality care during end of life
- Discussion of need of palliative care and end of life care ( care issues, place to die needs, discharge plan, short and long term care)
- Bereavement care, supporting family through grief and loss
- Provide quality integrated care and seamless transition across all setting including physical, psycho social and supportive measures (nutrition, spirituality and counselling)
